2023 Community Grant Application
To streamline and efficiently process applications, the Community Grant Program is requiring that applicants submit the following two (2) documents as part of their application. Please email
CommunityGrant@sdcda.org
with any questions and/or concerns.
2. California Form 590
4_590 Form 2022.pdf
Withholding Exemption Certificate, is a certification of residency and/or permanent place of business in California

Applicants must submit the Community Grant Program Application and all supporting documents in Adobe PDF file format. Additional documents submitted as attachments, other than the ones listed above, will not be reviewed.
Please note that all grant recipients are required to attend a webinar on the Community Grant Program’s reporting requirements and reporting portal. The date will be provided to grantees on a future date.
